The privacy of personal health information is a fundamental right of health care in Canada.

The physicians and staff of this clinic are committed to protecting your privacy.

We will:

  • keep accurate records of your health information and will follow all legal requirements for the security, retention and destruction of these records.
  • only collect information required for your care and treatment. We may share this information with other health professionals and hospitals who are involved in your care, on a "need to know" basis.
  • give you access to your records and, if requested in writing, make a copy of your information for you at a reasonable cost. In unusual circumstances you may be denied access to your records, but only if providing access would create a significant risk to you or to another person.
  • meet legal requirements to disclose medical information:
    • to agencies who pay for the care we provide to you.
    • when required by law.
    • in response to an investigation by the College of Physicians and Surgeons.
  • honor your right to request correction of errors to your records.
